info@med-integro.eu (from now on also referred to as "personal data controller"), informs the interested person about the fulfilment of information obligations following Art. 13 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016, of the protection of natural persons in connection with the processing of personal data and of the free circulation of such data, which repeals Directive 95/46/EC (from now on also referred to as "the Regulation") and Article 19 of the Act No. 18/2018 on Protection of Personal Data (from now on also referred to as "the Act on the Protection of Personal Data") by providing the information specified below.
Interested person, following Art. 19 part 2 point f) of the Act on the Protection of Personal Data, the data subject is obliged to provide personal data for the purpose specified below, otherwise it is impossible to conclude a contract for the provision of services towards which the interested person has expressed interest. When providing services, certain regulatory acts (for example, Act No. 404/2011 on Residence of Foreigners) require the provision of correct personal data, otherwise it is impossible to provide the client with the stipulated services.

2. Legal basis and purpose of personal data processing2.1 Purpose: concluding a contract and pre-contractual relationsLegal basis:Article 6 Part 1 (b) of the Regulation or Article 13 Part 1 (b) of the Act on the Protection of Personal Data: processing of personal data is necessary for the performance of a contract to which the interested person is a party or for the performance of a pre-contractual measure at the request of the interested party.
The controller hereby informs the client as the subject of personal data that in the process of concluding the contract, personal data will be processed without the consent of the subject of personal data, since the processing of personal data will be performed by the controller within the framework of pre-contractual relations and is necessary for the conclusion of a contract for the provision of stipulated services.
2.2 Purpose: sending newsletters, news, etc.Legal basis:Article 6 Part 1 (a) of the Regulation or Article 13 Part 1 (a) of the Act on the Protection of Personal Data: the interested person has consented to processing their personal data for at least one specific purpose.
Hereby, the controller informs the client as an interested person that if they wish to receive information about changes in providing the requested services in the system, information bulletins, newsletters, etc., without concluding a contract for the provision of services with the controller, the controller will process their e-mail address, to which the requested information will be sent within 48 months.
The controller declares that it will process personal data in good faith and act in a manner that does not conflict with the Act on the Protection of Personal Data and the Regulation.
3. Period of personal data retentionThe period of personal data retention for the purposes specified in clause 2.1 is a fixed period determined by law for the realization of rights and obligations arising from the concluded contract. It is impossible to determine the period in advance since representation or defence in court can last several years.
4. Rights of the interested personInterested person has the right to:
- request from the controller access to personal data relating to the data subject;
- correct personal data;
- have personal data deleted or limit its processing;
- object to the processing of personal data, as well as the transfer of data;
- withdraw your consent at any time;
- file a lawsuit to initiate proceedings following §100 of the Act on the Protection of Personal Data.

info@med-integro.eu or in writing to the postal address of the registered office of the operator. The controller must process the data subject's request regarding the above-mentioned rights within the time limits established by law.
